EPC Program (Individual Allied Health Services)

referral_imagePatients referred under this Medicare funded service can access up to five Exercise Physiology consultations per calendar year.

Patients are eligible for this service if they have a chronic medical condition and complex care needs.

A chronic medical condition is one that has been (or is likely to be) present for 6 months or longer. It includes conditions such as diabetes, cardiovascular disease, obesity, arthritis and musculoskeletal conditions.

consulting a doctorPatients have complex care needs if they need ongoing care from a multidisciplinary team consisting of their GP and at least 2 other health or care providers.

To be completed by referring GP:

  • GP Management Plan (item 721 or review item 732) AND Team Care Arrangements (item 723 or review item 732)
  • GP has contributed to or reviewed a multidisciplinary care plan prepared by the patient’s aged care facility (item 731)

For patients with type 2 diabetes, these 5 services are in addition to the Medicare Group Service Items (initial assessment 81110 and 8 exercise classes 81115)
